1. If you choose to complete the form above, please DO NOT enter your email address as your Client's email address.
2. Referrals are valid for 90 days from the date of acceptance.
3. After the Tenzo contract is signed, Tenzo will pay a referral commission per the terms of the Tenzo Referral Agreement.
4. Commissions are paid out quarterly.
